New Patient Registration

area mapIf you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so it would help if you could provide proof by way of a recent utility bill along with photo I.D.

To find out if your postcode is within our Practice area click here to enter it into our checker.

You will need to complete Form 1 (GMS1)    AND    Form 2a or 2b (new patient questionnaire) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Forms

A completed Form 1 (GMS1)     AND     Form 2a or 2b (new patient questionnaire) is required for each individual registering with the Practice. Once you have completed both Form 1 and Form 2a or 2b please bring them to us along with proof of address and photo I.D.

Form 1 (GMS1)

Form 2a (Patient Questionnaire 0-16 years)

Form 2b (Patient Questionnaire 16 years and over)

Medical treatment is available from the date of registration. Please contact reception for further information.
